I hereby acknowledge the above-name Participant as myself, being of at least 13 years of age, have my parent(s) permission to participate in and/or to attend WIN-NextGen™ programming (the “Program”).  I further acknowledge WIN-NextGen™, Inc., its affiliated individuals,  consultants, organizations, companies and their agents, successors and assigns, shall have the irrevocable right with respect to the Program and participants thereof, to film, videotape, photograph, and record the name, address, biographical data, photo, caricature, likeness, appearance, performance, voice and other resemblance (“Likeness”), and to use the results and proceeds thereof in connection with television commercials, article, books, radio broadcasts, audiovisual works and derivative works including, without limitation, the rights to edit, copy, exhibit, transmit, perform, copyright, reproduce, display, publish, license, sell, and otherwise use all or any portion thereof in any and all media, whether now known or hereafter developed, throughout the universe in perpetuity.  The rights of publicity provided herein shall not be contingent upon any conditions but solely on participation in the Program.  I agree to allow WIN-NextGen™ to release such pictures for publicity purposes. I further agree that, WIN-NextGen™ may share your personal information with trusted third-party strategic partners to deliver targeted outreach, advertising, promotions, and other relevant communications based on your interests and demographics, provided that such sharing is done in accordance with applicable laws and with your prior consent where required.
